Receipts of Services (superbills) are provided upon request for you to submit to your insurance for potential out of network benefits.

How much do I pay with using insurance?

Each insurance plan is different.  Please verify your benefits and coverage with your insurance company.  Here are the steps:​

  1. Call the phone # on the back of my insurance card

  2. Ask: "If Fiona Chen, LCSW is covered with my insurance plan?"

  3. Ask: "I will be billed 90791 (assessment, 1st time only), and 90837 (therapy, ongoing).  How much do I pay? " 

Note:

  • If I am in a deductible phase, I will be paying the full fee (according to the insurance contract) until my deductible is met.  Then, insurance will be responsible to some/all of the cost.

  • If I have a copay, I will pay a fixed amount of fee for each session.

  • If I have a coinsurance, I will pay a % of the fee for each session. (I may or may not need to meet my deductible.)
